Prevent: Are you eligible?

To participate, you cannot have a diagnosis of type 1 or 2 diabetes and must:

  • Be 18 years or older

  • Have a BMI > 25, or 23 if of Asian descent. Find out your BMI at choosemyplate.gov/tools-BMI

  • Have at least one of the following:

    • Fasting blood sugar of 100-125 mg/dL (110-125 mg/dL for Medicare members)

    • Hemoglobin A1c of 5.7%-6.4%

    • History of gestational diabetes**

    • Diabetes risk score > 5** (Risk Test at info session or www.doihaveprediabetes.org)

**Medicare members must meet blood test criteria within previous year regardless of risk score or history of gestational diabetes.

Prevent is currently open to eligible members & dependents of:

  • Providence Health Plan Medicare Advantage plans

  • Providence Health Plan for Providence caregivers

  • Providence Health Plan for Intel employees

  • Medicare A & B Plans

Covered in full with no co-pay or deductible for those who qualify under these insurance plans. Out of pocket payment options may be available for those that qualify and do not have insurance coverage for the Diabetes Prevention Program.
